Silverchain Hospital at the Home (HATH) Services

Silverchain Hospital at the Home (HATH) Services

The way that Royal Perth Hospital (RPH) delivers home care to our patients has changed.

The Hospital in the Home (HITH) service has transitioned to Silver Chain’s Hospital at the Home (HATH) service.

This program is funded by WA Health and is free for clients with a referral.

Silver Chain is a not-for-profit community health and care organisation that supports people to live independently by offering a range of services to help people in their homes.

These services include specialist nursing, palliative care, home care and support services, hospital in the home and allied health services. 

Short term acute services

Silver Chain provides short term acute home hospital services to patients across the Perth metropolitan area for a wide range of medical conditions.

Patients are either seen in their own home or in a Silver Chain clinic.

Treatment types include:

  • intravenous antibiotics and re-hydration therapy via PICCs and infusor ports
  • all interventions for patients in the immediate post discharge period
  • complex wound care.

Home Antibiotic Service (HABS)

The Home Antibiotic Service at RPH is supported Monday to Friday by a clinic staffed by a Silver Chain nurse.

Post-acute services

Silver Chain provides post-acute services such as wound care, continence management and stoma care.

Home support

A range of aged care services are available through Silver Chain.

Some patients may need to be referred to the Regional Assessment Service before they can start receiving aged care services through Silver Chain.
